Friends of Payton raises funds for Walter Payton College Prep to support programs and services not funded by Chicago Public Schools but that are nevertheless critical to the educational experience of Payton students.

• Location: Diversey Event Space / 2500 N Cannon Drive / Chicago
  
• Registration fee is $20. Each registration is for single participant and includes a Payton 5K t-shirt and numbered race bib.
  
• Race t-shirts and bibs can be picked up at Walter Payton College Prep (1034 N. Wells St.) on Thursday, May 19 and Friday, May 20 (7:30am-3:30pm both days), and on race day at 9:30am, at Diversey Event Space.

Race Release & Waiver

In consideration of being allowed to participate in any way in the program, related events and activities, I acknowledge, appreciate and agree that:

1.   The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death.

2.   I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releases or others and assume the responsibility of my participation.

3.   I willingly agree to comply with terms and conditions for participation. If I observe any unusual significant hazard during my presence or participation, I will remove myself from participating and bring such to the attention of the nearest official immediately.

4.   I, for myself, and on behalf of my heirs, personal representatives and next of kin, hereby release, indemnify and hold harmless Walter Payton College Prep, Payton Boosters Association Inc., Payton Alumni (PACE), Friends of Payton (FOP), Walter Payton College Prep Board of Directors, all officers, agents, employees, other participants, sponsors, advertisers and if applicable, owners and lessors of premises used to conduct the event, from any and all claims, damages, losses and liability arising out of or related to any injury, disability or death I may suffer, or loss or damage to the person or property, whether arising from the negligence of the releases or otherwise, to the fullest extent permitted by law.

Further, I hereby release the Chicago Park District, and members of the Chicago Park District Board, and the officers, agents, and employees of the Chicago Park District, individually and collectively from any aforementioned liability, costs, expenses, and responsibility.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY PARTICIPATING IN IT, AND ACCEPT RESPONSIBILITY FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

BY SUBMITTING ENTRY INTO THE WALTER PAYTON COLLEGE PREP 5K, I ACKNOWLEDGE (OR A PARENT OR ADULT GUARDIAN FOR ALL CHILDREN UNDER 18 YEARS) HAVING READ AND AGREED TO THE ABOVE RELEASE AND WAIVER.

FURTHER, I GRANT PERMISSION TO ALL THE FOREGOING TO USE MY NAME, VOICE AND IMAGES OF MYSELF IN ANY PHOTOGRAPHS, MOTION PICTURES, RESULTS, PUBLICATIONS OR ANY OTHER PRINT, VIDEOGRAPHIC OR ELECTRONIC RECORDING OF THIS EVENT FOR LEGITIMATE PURPOSES.

Payton Boosters Association (PBA) is dedicated to raising school spirit, fundraising for Payton's interscholastic teams, and celebrating and publicizing the achievements of incredibly talented and dedicated Grizzly students. Payton Alumni Community & Endowment (PACE) works to build the community of Payton's alumni, and oversees the endowment that supports the school.
