It is the holiday season! Shop for gifts while you support Lake Health Foundation.

 Your purchases will not only make great gifts for your family, friends, and yourself but will also benefit the Lake Health Cancer Prevention Program.  
 

We all know someone who has had to fight cancer. Early detection is key to prevention and treatment. The Lake Health Cancer Prevention Program aid our family and friends in winning the battle against cancer. Join us in ending cancer when you Shop For A Cause.

How do I receive the items I win?

Items can be picked up at the Lake Health Administration Building in Concord Township on the following dates with proper photo identification:      

·         Tuesday, Nov. 16th, 10:00 am-5:00 pm

·         Wednesday, Nov. 17th, 10:00 am-5:00 pm

·         Thursday, Nov. 18th, 10:00 am-5:00 pm

If you need other arrangements, please work with our Lake Health Foundation Office by emailing [email protected] or calling 440-354-1934.  You won't even have to get out of your car!

All my items are gift cards/certificates.  Do I still need to pick them up?

If all of the items you won are gift cards or gift certificates, we would be happy to mail them to you at the address provided on your profile by certified mail.  However, Lake Health Foundation is not responsible for items not received.

I was the lucky winner of the 50/50 Raffle / Cashpalooza Raffle , how will I receive the funds?

The winner(s) of the 50/50 raffle and Cashpalooza raffle will be required to complete the proper tax form and return it to Lake Health Foundation. Once received, we will request a check and mail it to the address provided by the winner on the tax form.

Can I buy packages that include alcohol if I am under 21 years of age?

You must be 21 years of age or older to purchase items where alcohol is included.

If I am not looking to purchase gifts right now but still want to support Lake Health Foundation, what are my options?

We have included a donation button that you can use to support the Lake Health Cancer Prevention Program. We thank you for all gifts! You can also share the link LHshop21.givesmart.com with your family and friends.

 Please pay careful attention to all conditions, restrictions, and expiration dates on items and services. Contact businesses for their Covid-19 safety policy.
