Self-Pay Policies

• I understand that this Self Pay Package is NOT valid for AZOPT Kids Place
• I understand any extra services, including dry needling, will have additional costs
• I understand that this Self Pay Package offers visits that would be at a discounted rate; therefore, these visits cannot be submitted to my insurance company by Arizona Orthopedic Physical Therapy, PLLC
• I understand that I can submit the visits to my insurance company for personal reimbursement, but that my insurance company may not reimburse me at all.
• I understand that my insurance company may not reimburse me in full for the package; Arizona Orthopedic Physical Therapy, PLLC will not reimburse the difference.
• I understand that I must schedule my appointments and that I will NOT receive a refund for the sessions not used. I understand that I have 120 days after my first visit to use all of my visits.
• Appointments cancelled within 4 hours of the appointment will count as a late cancel and cannot be rescheduled.
