QDP is an annual reduced-fee savings plan for families and individuals that allows all
QDP Members to receive quality dental services at greatly reduced prices. Unlike
conventional insurance plans, with QDP there are no deductibles, no yearly maximums,
and no waiting periods to begin treatment. QDP benefits coverage begins immediately
on plan registration.

Benefits include:
o Free simple teeth cleanings (up to two per year, in the absence of periodontal
disease/infection)
o Free exams (up to two per year, including initial exam)
o Free x-rays
o Free fluoride to age 16 (up to two per year)
. Half off sealants on permanent molars to age l6
o Free take-home teeth whitening (bleach refills after initial appointment will be
20% off.)
. $25.00 emergency exam
o A 20% discount on all dental procedures paid in full
o A 10% discount on all dental procedures paid with payment plan
o A l0% discount on orthodontic procedures paid in full
o A 50% discount on orthodontic procedures paid with payment plan

A QDP Membership is $249 for an initial plan member... and only $199 for each
additional family member. Eligible family members include spouse and dependent
children under the age of l9 (up to age 23 if dependent child is a full-time student). All

plan membership fees are payable in full at time of registration and are non-
refundable. Plan duration is for one year from registration date. All QDP Membership

fees are due and payable at the time of registration. Interest-free payment plans of 3, 6
and 12 months duration are available on request with approved credit. Repayment
duration is based on service totals. When a payment plan is used, your QDP Member
Discount will be reduced to 10oh. A missed appointment fee of 25Yo of treatment total
will be charged for all missed dental appointments. Please notify our office at least 2 hours in advance if you must change a scheduled appointment.
