NAMI Step Forward Walk 2024

Thank you so much for your interest in participating in the NAMI Louisville's Step Forward for Mental Health walk! We are so excited to have another year of bringing our community together to raise awareness of mental health and those who have been impacted by mental illness.

What to expect:

· The walk is roughly 2 miles, so make sure you bring a water bottle to stay hydrated. We will have water stations throughout the walking path where you can fill up your bottles.

· There will be free parking available around the waterfront park, with some options for paid parking closer to the location. We will be sending out a map with further instructions on where to go closer to the event.

· Food will be available for purchase at the event for those who are interested.

· Our silent auction items will be available for purchase with cash or card, but you must be present to be eligible to receive your item.

TEAM INSTRUCTION

If you are a part of a sponsor team and purchasing a ticket, proceed as normal. Be sure to enter your company or team name in the bottom of the "Purchaser and Attendees" section in the ticket form, in the text box titled "Company or Group Name". 

If you are part of a sponsor team and claiming a free ticket, DO NOT BUY A NEW TICKET! Find your NAMI Louisville contact from your organization and share with them your name, phone number, and email. They will get in contact with us to secure your ticket. 

THE NAMI PHOTO RELEASE FORM

I hereby grant NAMI Louisville permission to use my likeness in a photograph, video, or other digital media ("photo") in any and all of its publications, including web-based publications, without payment or other consideration.

I understand and agree that all photos will become the property of the NAMI Louisville and will not be returned.

I hereby irrevocably authorize the NAMI Louisville to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo.

I hereby hold harmless, release, and forever discharge the NAMI Louisville from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

Purchase of ticket for "NAMI Louisville Step Forward for Mental Health Walk 2024" is an implicit agreement of agreeance to the aforementioned photo release. 

I HAVE READ AND UNDERSTAND THE ABOVE PHOTO RELEASE. I AFFIRM THAT I AM AT LEAST 18 YEARS OF AGE, OR, IF I AM UNDER 18 YEARS OF AGE, I HAVE OBTAINED THE REQUIRED CONSENT OF MY PARENTS/GUARDIANS.
