I consent to allowing Pro-Life Wisconsin to capture and use my personal data. I understand and have read the Terms of Service and understand that I may revoke this consent at any time.

I understand that participation in trip activities could involve risk of physical injury, illness, death or property loss, and despite safety precautions, Pro-Life Wisconsin ETF and Pro-Life Wisconsin Inc., cannot guarantee safety thereof, as all risks cannot be prevented. Pro-Life Wisconsin ETF and Pro-Life Wisconsin Inc., does not provide health and accident insurance for trip participants, and I understand that any medical expenses, property loss, or other personal expenditures that result during or from this travel/trip, are to be borne by the student/participant, or by their parent or guardian (if student/participant is a minor).

I also hereby consent, give authorization to, and release from liability; trip leaders to secure any emergency medical treatment in event I am unable to, and I agree to be responsible for the costs thereof. I also authorize the release of relevant health records to health care professionals and/or trip leaders for the purposes of providing me with medical treatment or for the purposes of securing any emergency care or medical treatment.

In consideration of the opportunity afforded, with full knowledge and acceptance of the risks associated with this trip and any recreational activities taking place on this trip; and with full understanding of the above issues/conditions and risks, I hereby release, indemnify and hold harmless Pro-Life Wisconsin ETF and Pro-Life Wisconsin Inc., its staff, trustees, officers, volunteers, and agents from all form and manner of risks associated with this trip.

I hereby grant permission to PLW ETF and PLW, Inc. to use photographs and/or video of me taken January 18-24 2023 for the March for Life DC event, in publications, news releases, online, and in other communications related to the mission of Pro-Life Wisconsin.
