Dog must have a completed profile with Biscuits & Bath including:

• A copy of a vaccination record proving up to date vaccines for Distemper / Parvo, Rabies and Bordetella signed by the vet you administered the vaccines
• A copy of the RABIES certificate with the lot and expiration of the vaccine given signed by the vet who administered the vaccine
• If 8 months or over, must be spayed or neutered
