I agree to allow Cassata Catholic High School to use my personal data (name, email address, and phone number) for the purpose of registration and communication regarding Cassata's fundraising campaigns.

If I choose to donate or participate in fundraising activities for Cassata (including silent auctions), I consent to allowing Cassata Catholic High School to capture and use my financial data for the sole, and time-limited purpose of processing my financial contribution or payment.

I also have the option to choose whether or not to provide my physical mailing address to receive Cassata communications, such as donation acknowledgements, receipts, and annual reports, and information about future Cassata fundraising campaigns. 

I understand and have read the Terms of Service and understand that I may revoke this consent at any time.
