I consent to allowing Change is Simple, Inc to capture and use my personal data. I understand that Change is Simple will not share or sell my personal data. The data collected through GiveSmart will be used solely for transactions and communication related to the Change is Simple Fall Event and Change is Simple, Inc.
I understand and have read the Terms of Service and understand that I may revoke this consent at any time.
