Item Donation Form

Thanks for your interest in donating to our Silent Auction! Here are the types of items we are accepting:

- Items with a market value greater than $50
- Gift cards, jewelry, accessories, hotel stays, wine, skincare, etc.
- No art of any kind and no consignment.
- No services that require a partial payment. For example, no $1,000 towards home decorating or $500 off tutoring package. Services must be completely paid for by the auction price.
- Gifts cards for cosmetic or dermatology services are welcome. It is preferred that they are for a monetary value versus a specific procedure.
- Please Note: If the item does not sell, ICU baby will use the item in another fundraising or NICU family event unless the donor would like the item returned.
