Item Donation Form

THANK YOU! for donating to the 32nd Annual Anderson School Auction.

READ CAREFULLY!!!

Please enter all the details for your donation below. Attach any photos of donation, logos, etc.

*NOTE* REQUIRED: In the description box BELOW please include the following:

1) DESCRIPTION of the item

2) Any RESTRICTIONS (ie. expiration dates) if applicable

3) If you are NOT the donor, (log out of your account) and fill out DONORS information on the form and ADD "SOLICITED BY: YOUR NAME + EMAIL" in the description box.

4) How you will SUBMIT your donation in ONE of the following ways:

• UPLOAD - an image of e-gift certificates/vouchers now

• ASK US to GENERATE a CERTIFICATE based on the info provided

• MAIL - voucher/gift card (Paper items only) to: PS 334, The Anderson School Auction, Planetarium Station, P.O. Box 339, NY NY 10024
• PHYSICAL ITEM - reach out to [email protected] coordinate delivery for larger non-paper physical items
