By selecting 'Yes' on the registration form, I give my permission for Corewell Health Foundation Southwest Michigan, and its parent organization, Corewell Health, to publish or otherwise utilize my statements, written or verbal, about the Gala 2023 Coming Together event, or Corewell Health.  I likewise authorize and give permission for Corewell Health Foundation Southwest Michigan or Corewell Health to use my name, image, likeness, written or verbal statements, interviews, or other audio or visual recordings in any marketing materials, promotional materials, or other corporate publications, in any media format or platform.  These authorizations are provided without right to or expectation of compensation, and I agree that Corewell Health Foundation Southwest Michigan or Corewell Health will exclusively own the copyright to any materials that are generated consistent with these authorizations.
