By submitting this form, I authorize Second Chance Rescue, Inc. to implant a microchip in my pet. I entirely release Second Chance Rescue, Inc., its successors, assigns, and its officers, employees, representatives, partners, and volunteers from any and all claims and liabilities, demands, damages, causes of action, and possible actions whatsoever that may be sustained by any person or property from an act by our pets or actions arising from microchipping.

Furthermore, I accept that my pet may experience side effects after microchip insertion, that I must examine the injection site over the next week, and that I must check the microchip's location annually.
I understand that if my pet develops any unanticipated problem following the microchip insertion, I'm solely responsible for seeking veterinary care as needed or directed, including any necessary veterinary care and the costs incurred.
Second Chance Rescue Inc. retains the authority to refuse services if a dog is determined to pose an immediate threat to the facility's staff or other animals.
